In the era of electronic medical charts, you would think managing health data would be easy. In some ways it is, in others it's not. My Paper Chart is your private hand-written health record.
There was a time when a handwritten medical record was common practice – every patient had their own “medical chart” which was a binder or folder where the medical clerk would place progress and diagnostic reports into so that regardless of who was reviewing, they could see everything that happened up until that point in order to provide the best continuity of care.
Now, all your data goes into your electronic medical chart which contains your digital health record that can be viewed not only by the practitioners who provide care, but also you, the patient.
If you see practitioners that are part of different hospital networks, there is a good chance you have more than one “mychart” account to log into. Add small independent medical practices that provide their own portals and you can easily have 3-6 different portals. This may not be a problem for some, because they only access the account once a year after annual wellness checks. For anyone managing chronic conditions or caring for others, keeping track of multiple portal logins can be confusing and exhausting.
Yes, an electronic chart is a better way to share information transparently between practitioner and patient, however, life happens fast and not everyone is tech-savvy so sometimes it's easier to write stuff down. Even if you aren't managing a chronic condition, having a handwritten account of doctor visits and health vitals can prove to be valuable later for recalling what happened, the course of action taken, and monitoring health over time.
My Paper Chart is for anyone looking to create a hand-written medical record to track important health data. Caregivers and those managing chronic conditions could also benefit.
My Paper Chart is your personal health information in one place – on paper. It's a bridge. This is not intended to take the place of a digital portal provided by medical and health practitioners. It is intended as a supplemental add-on.
Even in the future, when we will have an interoperable digital chart, keeping hand-written logs and records can be helpful in the event there are power or internet outages.
My Paper Chart is meant to be used to record your health information. Consideration should be given to make sure it is stored securely in a private place where only necessary care team members can access.
Right now. We need a bridge between the old paper medical records and the future where it will be a seamless experience across all organizations.
While the future promises a more interoperable experience when it comes to managing our health information, at the moment, we are not there. Keeping a hand-written record makes it easy to access information without needing to log into a portal.

The books are absolutely fine to use on their own, but printable pages allow an opportunity to create a personalized chart by combining your hand-written data with printed pages from your digital portal provided by practitioners.
With our printable pages, choose whether to use a folder or binder, then figure out how to organize it. Learn More
